  3. Warning:<\/b> Sudden weight gain? This could indicate heart failure (water retention). The system immediately sends a push message to the responsible nurse or doctor. <\/p><\/li><\/ol>

    The result: intervention before<\/i> Mr. M\u00fcller has to go to hospital. Reactive firefighting tactics become proactive prevention<\/b>. <\/p>

    Why the “Hospital at Home” model is the future<\/h2>

    We are heading towards a demographic reality in which there will simply not be enough places in care homes. “Outpatient before inpatient” is no longer just a political slogan, it is a necessity. <\/p>
